Integration of Health and Social Care

By working together we believe that we can make Powys a better place to grow up, work, play and grow old, to thrive.

Our Goals

To enable people to take more responsibility for their own health and well-being

To enable people to stay well in their own homes and communities as far as possible

To focus on prevention

To provide access to timely & appropriate secondary care if required while minimising the risk of re-admission

Integrating services means we can respond better to people's care needs arising from an ageing population and increases in the number of people with multiple long-term conditions.

By working closely together we can provide services that enable people to live more independent, fulfilled lives.

When people do need us, we will provide responsive services in a way that meets their particular needs.

What are we doing to work more closely together?

Our first priority is to create health and social care teams in the community to include district nurses, speech and language therapists, social workers, physiotherapists, occupational therapists.

These teams will work closely with local GPs to offer care in or as close to people's homes as possible
