CHC Foundations Training (Continuing Health Care)

For all Social Workers in Adult Social Care

Aim

To provide knowledge of The 2014 Framework for Continuing NHS Healthcare (CHC) to all staff who utilise this process with patients within their care. To look at the Welsh Governments policy for eligibility for CHC, and the responsibilities of NHS organisations and local authorities. Looking at the process for the NHS, working together with local authority partners, to assess health needs, decide on eligibility for CHC and provide appropriate care. Participants will be able to provide a clear and transparent rationale to support the decision making process.

We will go through the Decision Support Tool and simplify the completion.

Key Learning Objective

By the end of the course, the delegate will be able to:

  • Apply A Primary Health Need approach when determining CHC eligibility.
  • Provide the evidence to support decision making and know what supporting information is required.
  • Know how to complete the documentation.
  • Understand the CHC process and what it means for the individual, family and Health Board.
