Apply to become a Local Authority Governor

My local Councillor(s) is/are aware of my application and has endorsed this application to become a Local Authority governor.

I certify that I am not disqualified from appointment as a school governor for any reasons given in Schedule 5 of The Government of Maintained Schools (Wales) Regulations 2005 (see LA Qualifying Criteria and Disqualification).

If appointed to a governing body I agree to inform the clerk to governors if I subsequently become ineligible for any of the reasons stated above, or as a result of any future regulations.
