Pathway 3 - Complex Support

If the hospital feel that longer term complex care is required, they will refer to social care via ASSIST for screening and allocation to appropriate team within Powys Adult Social Care.

It will have been identified that you have significant health and/or social care needs which need long term support. We will work with you to identify the most appropriate place for you to receive this support which may involve a care home.

Where possible we will always aim home first if safe and appropriate to do so.
