Adults at risk multi-agency referral form (MARF)

Important Information
1. If there are immediate concerns for an adult at risk, a referral should be made immediately by telephone to the assessment service / Duty Team.
  
  In such cases, this form should then be completed and sent to the assessment service / duty team the same working day in accordance with All-Wales Procedures.
2. Guidance Notes
  
  An “Adult at risk” is a person aged 18 years or over whom:
  • Is experiencing or is at risk of abuse or neglect and
  • Has a need for care and support and
  • As a result of those needs is unable to protect himself against the abuse or neglect or the risk of it
  
  Adults at risk may have or may lack mental capacity to make specific decisions. The Mental Capacity Act 2005 specifies that:
  “A person lacks capacity in relation to a matter if at the material time he is unable to make a decision for himself in relation to the matter because of an impairment of, or a disturbance in the functioning of, the mind or brain”
  
  A person is assumed to have capacity unless he/she is assessed as unable to do any one of the following:
  • Understand the information relevant to the decision; or
  • Retain information; or
  • Use or weigh that information as part of the process of making the decision; or
  • Communicate their decision (whether by talking, using sign language, writing etc)
