Pathway 2- Short Term Supported Facility

If the hospital feel that longer term care is required, they will refer to social care via ASSIST for screening and allocation to appropriate team within Powys Adult Social Care.

This may involve an interim bed in a care home whilst you regain some of your independence before returning home or where we can undertake further assessments in a more appropriate environment.

If you require long term care either via a package of care in the community or a residential placement will be assessed for following referral to ASSIST.
