All Wales Dementia Care Pathway of Standards
The All Wales Dementia Care Pathway of Standards were launched by Welsh Government in March 2021.
In November 2021 Powys held a conference to introduce the Standards locally to identify people from Statutory and Third sector organisations who were interested in being involved in progressing alignment to the Standards in Powys.
Four workstreams were developed:
- Community Engagement & Dementia Navigator
- Memory Assessment/Learning Disabilities/Mild Cognitive Impairment.
- Dementia Friendly Hospital Charter
- Learning and Organisational Development
*measurement is considered across all workstreams
