Have we made a difference?

Where you have a Care Plan, we will aim to regularly review it with you:

• To make sure it is still fit for purpose.
• To understand if it has made a difference.
• To identify if any changes are needed.

When things are getting better it's important to say so, but when they are not, we may need to make changes.
