Model of Practice

1024_helpathome
Our aim is to help you and the people who care for you to stay independent, safe and well so you can live the life you want to.

We will help you to identify your strengths and how to use these to overcome difficulties in your life.  Focussing on what is working well and building upon this to help you achieve your goals (outcomes).

Why do we do things this way?

It's the law. The Social Services and Well-Being Act (Wales) 2014 describes what we must do to support you and your carers.
