If you wish to see a prosecution and are willing to attend court as a witness, please include a witness statement.

Download and complete the Witness Statement Form below save it onto your computer then upload it with the form on this page. This will then be sent to the Police to provide a case for a prosecution, or by all means send it to the Police directly at contactcentre@dyfed-powys.pnn.police.uk
