Assist can provide both individuals and professionals with information, advice and assistance relating to Adult Social Care. This enquiry form allows you to share information with Assist team, who will then make contact with you to have a “What Matters” conversation.

A “What Matters” conversation is individual to all who contact Assist and should promote your wellbeing and outcomes. A “What Matters” conversation will consider:

• Your circumstances
• The outcomes you are looking to achieve
• What the barriers are to achieving this
• Your strengths and capabilities
• Any risks that need to be considered

Contact officers will help you explore information, advice and options are available to you.

*Professionals referring in on behalf of a person are expected to have completed the “What Matters” conversation prior to referral to establish outcomes.
