Support with Leaving Hospital
When it is time for you to leave hospital, a team of professionals from health and social care will speak with you to plan your discharge, we will call this your 'discharge pathway'.

Pathway 0 - Discharge
If you and/or the team of professionals decide you do not need formal support to go home, we may suggest a referral to other support agencies who can help maintain your independence at home.

Pathway 1 - Supported Home First
If you and the professionals decide that you need a little bit of extra support when you go home to get you back to being fully independent, this section will inform you of Powys Home First and Reablement support available.

Pathway 2- Short Term Supported Facility
If the hospital feel that longer term care is required, they will refer to social care via ASSIST for screening and allocation to appropriate team within Powys Adult Social Care.

If the hospital feel that longer term complex care is required, they will refer to social care via ASSIST for screening and allocation to appropriate team within Powys Adult Social Care.
